Collaborative Partnerships to Reduce Risk
The Plymouth County Hub, founded by a collaboration between Plymouth County Outreach (PCO), Beth Israel Deaconess Hospital in Plymouth and Police Assisted Addiction and Recovery Initiative (PAARI) with grant funding by South Shore Health, strives to fill the gap where there is an acute need. The Hub is dedicated to facilitating partnerships with community resources to identify individuals or families that meet criteria for high level of needs across multiple areas with the purpose of providIng linkages to services grounded in the Plymouth County community in order to reduce and prevent the risk of harm.
Ways We Help
The Hub seeks to build collaborative relationships between law enforcement and community resources to link at-risk individuals and families to supports and resources by hosting four area specific meetings every week. The four Hub areas are based on the four District Court regions in Plymouth County (Plymouth, Wareham, Brockton, and Hingham). The Plymouth County Hub is in the business of rehabilitating, enhancing, and saving lives. The work that Hub members do is aimed at providing a multidisciplinary approach to solving some of society’s biggest challenges.

Hub situations can be brought to our meetings through any provider affiliated with the Hub, including local public safety officers and public welfare organizations throughout the county.
