Plymouth County HUB strives to fill the gap where there is an acute need.

Plymouth County HUB is dedicated to facilitating partnerships with community resources to identify individuals or families that meet criteria for high level of needs across multiple areas with the purpose of providIng linkages to services grounded in the Plymouth County community in order to reduce and prevent the risk of harm.

Ways We Help

Connecting partner agencies throughout Plymouth County in 4 weekly meetings, the HUB seeks to build collaborative relationships between law enforcement and community resources to link "acutely elevated risk" individuals and families to supports and resources.  Our 4 HUBs are based on the 4 District Court regions in Plymouth County.

Plymouth County HUB is in the business of changing and shaping lives. The work we do at our Non-Profit Organization is aimed at providing a holistic approach to solving some of our society’s biggest challenges. We make sure our partners are empowered by creating opportunities for individuals and communities.

If you, or someone you know, is at risk for harm, please reach out to your community provider and ask about the HUB.  HUB situations can be brought to our meetings through any provider affiliated with the HUB, including local public safety officers and organizations that provide supports throughout the county.
